2. Description of Service

CogniBios is a wellness tracking application designed to help users monitor and improve their lifestyle habits based on the principles of lifestyle medicine. The App allows users to:

• Track meals, exercise, sleep, and stress levels

• Receive AI-powered health coaching suggestions

• Complete health assessments

• Schedule consultations with healthcare providers

3. Not Medical Advice

The information provided through CogniBios, including but not limited to health scores, AI coach responses, meal analyses, and wellness recommendations, is for general informational and educational purposes only.

This App does not provide medical advice, diagnosis, or treatment. The content is not intended to be a substitute for professional medical advice, diagnosis, or treatment.

Never disregard professional medical advice or delay seeking it because of something you have read or received through this App.

4. User Responsibilities

You agree to:

• Provide accurate information when using the App

• Use the App only for lawful purposes

• Not rely solely on the App for health decisions

• Consult with qualified healthcare providers for medical concerns

• Maintain the confidentiality of your account credentials

5. AI Health Coach Limitations

The AI Health Coach feature provides general wellness suggestions based on lifestyle medicine principles from the American College of Lifestyle Medicine (ACLM). These suggestions:

• Are not personalized medical recommendations

• Do not account for your complete medical history

• Should not replace professional medical consultation

• May not be appropriate for all individuals

6. Consultation Services

The App may facilitate scheduling of consultations with Dr. Anna Handa or other healthcare providers. These consultations are separate services and subject to their own terms and professional standards.

7. Subscription and Payments

Payments are charged to the user's iTunes Account at confirmation of purchase.

Subscription automatically renews unless auto-renew is turned off at least 24 hours before the end of the current period.

Your account will be charged for renewal within 24-hours prior to the end of the current period.

Any unused portion of a free trial period, if offered, will be forfeited when the user purchases a subscription.

User can manage or turn off auto-renew in their Apple ID Account Settings after purchase

Your use of the app is also governed by Apple Standard Licensed Application End User License Agreement.

Some features of the App require a paid subscription. By subscribing, you agree to pay all applicable fees. Subscriptions automatically renew unless cancelled before the renewal date.

8. Limitation of Liability

To the maximum extent permitted by law, CogniBios and its creators shall not be liable for any indirect, incidental, special, consequential, or punitive damages resulting from your use of or inability to use the App.

9. Changes to Terms

We reserve the right to modify these Terms of Service at any time. Continued use of the App after changes constitutes acceptance of the modified terms.
